Questionnaire for the Online Learning Agreement to collect the names of the responsible persons of your institution

Dear Partner,

this is a short questionnaire initiated by Paderborn University, Germany.

Soon we want to start to use the Online Learning Agreement, which you can find here: https://learning-agreement.eu/

We would like to make sure that every outgoing student fills in the correct name and email address for signing of the person who is responsible for the approval of their courses at their host university. 

We have developed this short questionnaire to collect the names and email-address(es) of the responsible person(s) at your university and for which field(s) of studies there are responsible respectively.

All in the questionnaire listed ISCED-codes are also listed in the Online Learning Agreement.

We would appreciate it if you would take a few moments to fill out the questionnaire.

If there is more than one responsible person at your institution, you can fill out the questionnaire as often as necessary.
